PROVIDENCE, R.I. (AP) - A review by Rhode Island’s child advocate has found that four children died in the past year despite warnings from family, friends, police and others.
The findings in a report released Thursday by the Child Fatality Review Panel call for an overhaul of the way the Department of Children, Youth and Families Child Protective Services assesses risk and investigates calls placed to its hotline.
The Providence Journal (https://bit.ly/2nIs5Ql ) reports that in one case, five calls were made to Child Protective Services in the six months leading up to the infant’s death. The callers reported parental substance abuse, neglect and concerns about the child being dropped repeatedly. The agency investigated one of the calls. A month later, the state closed the case.
The review found calls were consistently categorized in a way that failed to trigger an investigation.
